Registration Procedure
Online registration for the 50th Annual Meeting is encouraged. However, delegates may also register by completing and mailing or faxing the Delegate Registration Form or Group Registration Form. All registrations will be confirmed via email. The advance registration deadline is August 15, 2015. After this date, registrations will only be accepted onsite. No exceptions will be made.

The Annual Meeting runs from Wednesday, September 30 to Saturday, October 3 and includes admission to all general and concurrent scientific sessions, the Pre-Meeting Course on Wednesday, September 30, lunchtime symposia, breakfast, breaks, lunches, Half-day course on Thursday, October 1 and Welcome Reception & Opening Ceremonies on Wednesday, September 30.

B. Annual Meeting & Course
The Annual Meeting runs from Wednesday, September 30 to Saturday, October 3 and includes admission to all general and concurrent scientific sessions, the Pre-Meeting Course on Wednesday, September 30, lunchtime symposia, breakfast, breaks, lunches and Welcome Reception & Opening Ceremonies on Wednesday, September 30.

This DOES NOT include admittance to the Pre-Meeting Course on Wednesday, September 30 or the Half-Day Course on Thursday, October 1.

A la Carte Options
The following items may be added to the base registration for an additional fee. It is recommended these items be added during initial registration, as delegates will not be able to edit their registration online at a later time.
Farewell Banquet
Open to all registered delegates and registered guests. Tickets are $50 each and should be purchased in advance. A limited number of tickets may be available onsite, but SRS strongly urges delegates to purchase tickets at the time of registration.
The Farewell Banquet will take place at The Depot from 7:00-10:00pm on Friday, October 2.
Wine & Chocolate Tasting

Join us for a wine and chocolate tasting presented by The Wine Company. The tasting will feature Cabernet, Old World Syrah and New World Pinot Noir and will include a three-glass Riedel VERITAS set to take home. As an extra bit of fun and excitement the tasting will include a selection of three different chocolates which are selected to go with each wine further emphasizing the ways in which differing wines show distinctly not only in different glasses but also with an array of different food flavors. Guest Attendance
(Does not include session admittance.)
Guests must complete and submit a separate Guest Registration Form. Guest registration fees include access to the Guest Hospitality Program, entrance to the Welcome Reception on Wednesday, September 30 and the ability to register for social events including optional tours and the Farewell Banquet, for additional fees. An additional $50 charge applies for each ticket to the Farewell Banquet on Friday, October 2. For more information regarding the optional tours and the prices please click on the Tours Tab of the Annual Meeting website. Entrance to the scientific sessions is not included in guest registration fees. Only guests registered as Non-Member Non-Physicians will be allowed to participate in above scientific sessions and breaks. Sorry, no exceptions will be made.
Guest Attendance (Includes session admittance.)
Guests wishing to attend the Scientific Program should register as Non-Member Non-Physicians by filling out a separate Delegate Registration Form. Guests that register as a Non-Member Non-Physician will receive entrance to the General and Concurrent Scientific Sessions, Lunchtime Symposia, breakfasts, breaks, lunches, and the Welcome Reception on Wednesday, September 30. An additional $50 charge applies for each ticket to the Farewell Reception on Friday, October 2. Optional tours may also be added for additional fees, as indicated.

Substitutions
Name substitutions must be made to SRS in writing prior to August 15, 2015. Any name substitutions made onsite will result in a $100 processing fee. If the original registrant shows up at the meeting, he/she will be responsible for a new onsite registration fee.
Cancellations & Refunds
All cancellations must be received in writing. Delegates may fax their cancellation to (414) 276-3349 or email it to [email protected]. Delegates will receive a confirmation and refund within 14 days of receipt of their cancellation notice. SRS will grant a full refund, less a 10% processing fee, for cancellations received in writing on or before September 1, 2015. No refunds will be granted after September 1, 2015.
Wire Transfers
You may also pay for your Annual Meeting & Course registration through a wire transfer. Please note that some financial institutions charge a processing fee, which is deducted from the amount of the transfer. Partial payment may result in an incomplete registration. If you wish to pay with a wire transfer, please contact the SRS office for more information.
